Center Nurse

REPORTS TO:  Center Director

RESPONSIBILITIES

Included but not limited to:

  • Completing initial assessments and any ongoing assessments
  • Development and implementation of the treatment/care plan
  • Member assessments, discharge plan, and acting as the liaison with the member’s family, significant others, or care providers
  • Administration of medications or monitoring of self-administration of medication by Town Square members
  • Supervision of personal care provided by care providers
  • Maintenance of members’ health records, which includes:
    • Signed quarterly progress notes from each member of the multi-disciplinary team and outside consultants
    • Records of medications and treatments administered
    • Implementation of a nutrition program for participants, designed by food vendor and nutrition consultant
    • Aggregate and manage member documentation as required by MD
  • Other duties as assigned

Requirements

  • RN licensed in MD
  • 3 to 5 years of experience with assessment and treatment planning in adult senior care population
  • Successful results of background check, and where applicable, additional state specific regulatory requirements
